Job Summary

Achievers Spot is hiring Medical Coders to accurately convert patient medical records, diagnoses, procedures, and treatments into standardized medical codes used for billing, insurance claims, reimbursement, and healthcare documentation. The ideal candidate should possess a strong understanding of medical terminology, anatomy, physiology, and coding principles, along with excellent analytical and communication skills.

Key Responsibilities
Review patient medical records and clinical documentation for completeness and accuracy.
Assign appropriate ICD, CPT, and HCPCS codes for diagnoses, procedures, and treatments.
Convert healthcare information into standardized alphanumeric codes for medical claims and reimbursement.
Ensure coding accuracy while complying with healthcare regulations and coding guidelines.
Verify documentation to support assigned medical codes.
Identify missing or incomplete clinical information and communicate with relevant stakeholders when necessary.
Maintain confidentiality of patient health information in accordance with healthcare regulations.
Meet productivity and quality standards established by the organization.
Maintain accurate coding records and documentation.
Keep up to date with coding standards, medical terminology, and regulatory changes.
Collaborate with internal teams to improve coding quality and operational efficiency.
