Position Details

Job Title: Staff Nurse – Home Care
Location: Chennai
Experience: Minimum 1 Year
Qualification: GNM / B.Sc Nursing
Mandatory Requirement: Valid Nursing Registration Certificate

Key Responsibilities
Clinical Care
Provide direct and specialized nursing care to patients in accordance with established nursing protocols.
Assess patient conditions and develop individualized care plans.
Administer medications, insulin, and prescribed treatments.
Monitor vital signs, including blood pressure and blood glucose levels.
Observe patient progress and promptly report changes to physicians or nursing supervisors.
Perform non-critical medical and therapeutic procedures as authorized.
Home Healthcare Services
Deliver quality nursing care in patients’ homes.
Travel to patient locations and manage care plans as prescribed by physicians.
Assist with wound assessment, dressing changes, and infection prevention.
Support patients with personal hygiene and grooming needs when required.
Patient Education & Support
Educate patients and caregivers regarding treatment plans and healthcare practices.
Provide guidance to enhance specialized patient care and recovery.
Collaborate with families to ensure continuity of care.
Training & Professional Development
Conduct and support certification programs such as ACLS, PALS, and NALS.
Participate in nursing projects and clinical improvement initiatives.
Maintain and enhance clinical competencies within the specialty area.
Train healthcare staff on the proper use of equipment, supplies, and medical instruments.
Collaboration & Quality Assurance
Work closely with physicians, healthcare professionals, and patient care teams.
Participate in care planning, implementation, and evaluation processes.
Ensure all nursing interventions comply with organizational policies, procedures, and standards of care.
Perform additional duties as assigned by management.
Required Skills
Strong clinical nursing knowledge and patient assessment skills.
Experience in medication administration and wound care.
Excellent communication and interpersonal skills.
Ability to work independently in a home-care environment.
Strong documentation and patient-care management skills.
