Job Summary

We are looking for a dedicated and compassionate Home Care Nurse to provide high-quality, patient-centered nursing care in patients’ homes. The ideal candidate will be responsible for conducting clinical assessments, administering treatments, monitoring patient conditions, and educating patients and their families to ensure the best possible healthcare outcomes.

Key Responsibilities
Patient Care & Clinical Services
Visit patients’ homes as per the assigned schedule and provide nursing care according to physicians’ orders and established nursing protocols.
Perform comprehensive nursing assessments and monitor patients’ vital signs.
Administer medications, injections, intravenous (IV) therapies, and other prescribed treatments.
Manage catheters, feeding tubes, and other medical devices as required.
Provide wound care, post-operative care, and chronic disease management.
Monitor patient progress and promptly report any significant changes in condition to the physician or supervisor.
Patient & Family Education
Educate patients and family members regarding disease management, medication adherence, treatment plans, and healthy lifestyle practices.
Provide guidance and emotional support to patients and caregivers.
Documentation & Compliance
Maintain accurate, complete, and timely documentation of patient care activities.
Ensure compliance with nursing standards, healthcare regulations, and organizational policies.
Follow infection prevention, safety, and quality care protocols at all times.
Collaboration & Coordination
Communicate effectively with physicians, physiotherapists, and other healthcare professionals involved in patient care.
Coordinate care plans and treatment updates with the healthcare team.
Ensure proper handling, maintenance, and usage of medical equipment and supplies.
Professional Standards
Maintain patient confidentiality and uphold professional nursing ethics.
Deliver compassionate, respectful, and patient-focused care at all times.
Eligibility Criteria
Qualification
ANM (Auxiliary Nurse Midwife)
Experience
2 to 5 years of relevant nursing experience.
Required Skills
Strong clinical assessment and patient care skills.
Knowledge of medication administration and IV therapy.
Experience in wound care and chronic disease management.
Excellent communication and interpersonal skills.
Ability to work independently in a home healthcare environment.
Strong documentation and record-keeping abilities.
Commitment to patient safety, confidentiality, and professional ethics.
