Job Summary
Yakima Valley Farm Workers Clinic is seeking a compassionate Advanced Practice Clinician to provide comprehensive primary healthcare services to underserved and diverse patient populations. Working within a patient-centered medical home model, the clinician will collaborate with physicians, nurses, medical assistants, and other healthcare professionals to deliver high-quality, family-focused care while building long-term relationships with patients.
Key Responsibilities
Provide comprehensive primary and preventive healthcare services.
Diagnose, treat, and manage acute and chronic medical conditions.
Perform physical examinations and health assessments.
Order, interpret, and evaluate diagnostic tests.
Prescribe medications and develop individualized treatment plans.
Educate patients and families on disease prevention and health management.
Collaborate with multidisciplinary healthcare teams.
Maintain accurate and timely electronic medical records.
Promote health equity and improve access to quality healthcare for underserved communities.
Participate in quality improvement and patient satisfaction initiatives.
Required Qualifications
Nurse Practitioner
Master’s or Doctoral Degree in Nursing.
Current Nurse Practitioner licensure and certification.
Physician Assistant
Master’s Degree in Physician Assistant Studies.
Bachelor’s Degree in Health Science plus completion of an accredited two-year Physician Assistant training program (as applicable).
Current Physician Assistant licensure and certification.
Preferred Qualifications
Experience in Family Medicine or Community Health.
Experience serving underserved or migrant populations.
New graduates with strong clinical rotations in community health are encouraged to apply.
Bilingual skills (especially English/Spanish) are an advantage.
