Job Title: Home Health Medical Coder (ICD-10-CM)

Key Responsibilities
Assign accurate ICD-10-CM diagnosis codes based on clinical documentation for home health cases
Review and validate OASIS (Outcome and Assessment Information Set) data for consistency
Ensure proper coding aligned with PDGM (Patient-Driven Groupings Model) reimbursement guidelines
Maintain compliance with CMS, Medicare, and payer-specific regulations
Query clinicians for clarification on incomplete or unclear documentation
Perform coding audits and support quality improvement initiatives
Collaborate with clinicians, QA, and billing teams to resolve discrepancies
Stay updated with annual coding changes and regulatory updates
Ensure timely completion of coding tasks to meet billing deadlines
Required Skills
Strong knowledge of ICD-10-CM coding
Experience in home health coding and OASIS review
Understanding of PDGM reimbursement model
Familiarity with CMS and Medicare guidelines
Good analytical and communication skills
