Job Summary

As a Staff Nurse – Homecare, you will be responsible for delivering high-quality, patient-centered nursing care in patients’ homes. This role requires strong clinical skills, sound judgment, compassion, and the ability to work independently while collaborating effectively with families and multidisciplinary healthcare teams.

The ideal candidate will assess patient needs, develop individualized care plans, administer treatments, monitor patient progress, and provide education and support to patients and caregivers to promote optimal health outcomes.

Key Responsibilities
Patient Assessment & Care Planning
Conduct comprehensive nursing assessments in patients’ home environments.
Develop and implement personalized care plans in collaboration with physicians, healthcare professionals, patients, and family members.
Evaluate patient progress and modify care plans as necessary.
Clinical Care
Administer medications and treatments as prescribed by physicians.
Monitor patient conditions and promptly report significant changes to physicians or case managers.
Provide wound care, catheter care, IV therapy, and other nursing procedures as required.
Ensure safe and effective delivery of nursing services in accordance with established clinical protocols.
Patient & Caregiver Education
Educate patients and caregivers about disease processes, medications, treatment plans, and self-care techniques.
Provide guidance on maintaining health and preventing complications.
Support families in understanding care requirements and recovery processes.
Documentation & Compliance
Maintain accurate, complete, and up-to-date clinical documentation and reports.
Adhere to infection prevention and control standards.
Ensure compliance with healthcare regulations, nursing standards, and organizational policies.
Maintain patient confidentiality and professional ethics.
Collaboration & Support
Collaborate with physicians, therapists, social workers, and other healthcare professionals to optimize patient outcomes.
Provide emotional support and counseling to patients and families.
Assist patients and caregivers in accessing community resources and healthcare services when required.
Work Environment
Conduct home visits across various residential settings.
Flexible work schedules may include evenings, weekends, and holidays based on patient care requirements.
Work in diverse patient care environments requiring adaptability, professionalism, and resilience.
Required Skills
Strong clinical assessment and nursing care skills.
Knowledge of medication administration and home healthcare practices.
Experience in wound care, catheter management, and IV therapy.
Excellent communication and interpersonal skills.
Ability to work independently and manage patient care effectively.
