Job Summary
The Staff Nurse – Homecare is responsible for providing professional nursing care to patients in their homes. The role involves conducting patient assessments, developing individualized care plans, administering medications and treatments, monitoring patient progress, educating patients and caregivers, and collaborating with healthcare professionals to ensure comprehensive and compassionate care.
Key Responsibilities
Conduct comprehensive nursing assessments during home visits.
Develop and implement individualized patient care plans.
Administer prescribed medications and treatments safely.
Monitor patients’ health status and promptly report changes to physicians or case managers.
Perform nursing procedures such as wound care, catheter care, IV therapy, injections, and other clinical interventions.
Educate patients and caregivers on disease management, medications, nutrition, and self-care practices.
Maintain accurate clinical documentation and patient records.
Ensure compliance with infection prevention and patient safety protocols.
Collaborate with physicians, therapists, social workers, and other healthcare professionals.
Provide emotional support and guidance to patients and their families.
Promote patient comfort, dignity, and quality of life during home care.
