Job Summary

The Home Care Nurse is responsible for delivering professional nursing care to patients in their homes by assessing health conditions, implementing individualized care plans, administering treatments, monitoring recovery, educating patients and caregivers, and collaborating with the healthcare team to ensure high-quality patient care.

Key Responsibilities
Conduct comprehensive nursing assessments during home visits.
Develop and implement individualized patient care plans.
Administer medications and prescribed treatments safely.
Monitor patients’ health status and promptly report significant changes.
Perform wound dressing, catheter care, IV therapy, injections, and other nursing procedures.
Educate patients and caregivers on disease management, medications, nutrition, and self-care.
Maintain accurate clinical records and nursing documentation.
Follow infection prevention, patient safety, and quality care protocols.
Coordinate with physicians, therapists, case managers, and other healthcare professionals.
Provide emotional support and guidance to patients and their families.
Assist patients in accessing appropriate community healthcare resources when required.
