Home Care Nurse
Role Overview
The Home Care Nurse is responsible for delivering professional nursing care to patients in their homes. The role focuses on patient assessment, treatment administration, health monitoring, patient education, and coordination with healthcare teams to ensure high-quality care and improved patient outcomes.

DGNM / GNM (General Nursing & Midwifery)
B.Sc Nursing
Key Responsibilities
Patient Assessment & Care Planning
Conduct comprehensive nursing assessments in patients’ homes.
Evaluate patient health conditions and identify care requirements.
Develop and implement individualized care plans in collaboration with physicians, caregivers, and family members.
Clinical Care
Administer medications and prescribed treatments safely.
Monitor patient health status and document observations accurately.
Report significant changes in patient condition to physicians or case managers.
Provide specialized nursing procedures including:
Wound care
Catheter care
IV therapy
Injection administration
Basic clinical procedures as required
Patient & Caregiver Education
Educate patients and family members regarding:
Disease management
Medication usage
Lifestyle modifications
Self-care techniques
Promote patient independence and adherence to treatment plans.
Documentation & Compliance
Maintain accurate clinical records and reports.
Ensure proper documentation of assessments, treatments, and outcomes.
Follow healthcare regulations and organizational policies.
Safety & Infection Control
Adhere to infection prevention and control practices.
Ensure patient safety within the home environment.
Identify and address potential health or safety risks.
Collaboration & Support
Coordinate care with physicians, therapists, social workers, and other healthcare professionals.
Assist patients and families in accessing community resources and support services.
Provide emotional support and guidance to patients and caregivers.
Work Environment
Home visits across different residential locations.
Flexible working hours based on patient requirements.
May include evening, weekend, and holiday assignments.
Exposure to diverse patient populations and health conditions.
